Sir Charles Haddon-Cave presented a review of the UK MAA airworthiness system in his investigation into the RAF Nimrod XV230 loss and fatalities in Afghanistan. One of his statements within the report is:
“Most aviation organizations worth their salt espouse safety and airworthiness as their ‘highest priority’ and enunciate ‘principles’ to which they profess adherence. Translating these laudable aspirations into a practical reality, however, is easier said than done”.
He has offered four new principles as a foundation for a new airworthiness system:
